I am designing a steel plate that goes on top of steel screw anchor piles(3 nos.) The plate will be subjected to uplift force and I need to get the thickness of plate for this.

Where can I get "plate formulas" to find maximum bending moment and critical section for failure for this type of problem ( rectangular or square plate with concen. load). I tried to find something in Roark's formula book but of no use.

Please help.

I don't think you will find a "canned" solution for this sort of problem. Have you considered modelling it using finite elements or doing a yield line analysis?
 
In the site below look under Plates -> Simple bending -> Rectangular and choose the boundary condition closest to yours.
