Pre-tension rebar codes?

Hi all,

I have a question wrt a code combination that I've found on an old report (the company that wrote the report no longer exits) about a hollow slab (1.2x4.8x0.2 meters).
It states the following codes which refer to a allowable moment:
0.4.0.0. 48.26 kNm
0.5.0.0. 54.43 kNm
0.6.0.0. 68.02 kNm
0.6.4.0. 66.04 kNm
8.0.4.0. 111.33 kNm

I'm not sure what the code means, and I want to deduce the allowable bearing capability for this slab; or the applied pre-tensioning (than I can deduce the allowable tensile stress).
All help is welcome.
