Quality Issue/ Report

I am looking for a template that I can use for a part quality issue. I remember in the past when I worked Supplier Quality we had a document that allowed us to input a description, picture, corrective actions, etc. I cant find a non-conformance report that is basic, and will work internally to make special note of issues. Does anyone know where I could find something like this? This will be for internal reference only and not going outside our plant.

Sounds like a template that could be made in under 10 minutes in a myriad of applications... Word, Excel, etc.
