Could someone tell me if there exist a standard for the format of raw material test reports, which include the required data to be recorded on the Mill Test Report?

Or are these material test reports driven by each respective specification to which there are certified?

Each material specification specifies which tests are required. There are to many different material specifications with different requirements so there is no standard form. Some third parties like LLoyds and TUV use a kind of standard form where all kind of tests are tabilised. They cross out the none applicable ones and fill out the applicable ones.
 
While the base material specifications (API, ASME, EN, DIN, JIS, etc.)specify the required tests and information required, the format is up to the manufacture. After reviewing so many MTRs from so many different global suppliers, while I cannot speak for others, I often wish there was a standard format.

 
Material certification information or tests depend on the specifications the material is being made to. For instance, Type 303 manufactured to AMS 5640 only requires to list the chemical analysis of certain elements and to test hardness. Any other testing would need to be specified by the customer.
