Removal of vessel nameplates

Dear Friends,

One of our clients based in Kuwait has 'accidentally' removed nameplates from 5 vessel on site.
I have done some homework and have decided that we (vendor) will need to goto to site to witness the re-fixing of the nameplates along with the ASME AI.

My question is as follows:
1. Do we need to complete the Form NB-136 'Replacement of stamped Data Form'?
2. Can the AI Diary entry for the witness of re-fixing the nameplates be used as suitable evidence of the work and thus no need for Form NB-136?
3. Have I made a 'mountain out of a mole hill' here and no need for any documentation?(my understanding was that removing nameplates from vessels is a serious issue)
4. Any other recommendations/solutions?

Scott,
There are some very experienced vessel engineers on here who will hopefully resolve your issue for you.
One thing that stands out for me - why as a vendor do you care what the client has done ?
You have sold code compliant vessels (including nameplates) to a client.
What that client does with those vessels is now their responsibility - not yours.

At the very beginning of this issue, the [highlight #FCE94F]end-user[/highlight] of the refinery has requested my client (the EPC) to get approval from the ASME AI that it is acceptable for the EPC to simply re-attach the nameplates (the EPC prepared a form which the AI was supposed to sign)....

...The ASME AI has stated that he can not simply sign a form as acceptance of the nameplates being fixed, but must witness the fixing at site as per ASME. This is fair enough in my opinion.

From here it kind of snowballs to where many people are involved and the execution of the re-installment of the nameplates have become a hurdle.

As I stated in my OP, it may be that the ASME AI can simply witness at site and that is the end of the issue.

But I have read that a more formal/official document may need to be prepared.

To further complicate things, the vessels were fabricated in UAE and delivered to Kuwait, and the original fabricator in UAE is no longer in business.

ScottofArabia, removal of a nameplate is indeed serious business. My advice would be to closely collaborate with the AI. Do the things he requires, also collaborating with the other interested parties to insure concerns of each are met. But start with the AI. He signs off on it all.

CAP-21 CRITERIA FOR REAPPLICATION OF AN ASME CERTIFICATION MARK
1 After an item has been certified under an ASME standard, if the ASME certification mark (e.g. Code Symbol Stamp) becomes indistinct or the nameplate is illegible or lost, but traceability to the original certification can be established, the certification mark may be reapplied to the item.
2 Reapplication of certification marks shall only be permitted under the following conditions:
2.1 The owner has requested the reapplication.
2.2 Where applicable, the Jurisdiction has granted the request for reapplication.
2.3 The reapplication shall be performed by the original manufacturer of the Code item. Where responsibility to the original Code certification has been maintained, reapplication by a successor organization to the original manufacturer is permitted.
2.4 The reapplication shall be authorized and witnessed by an Inspector from an ASME accredited Authorized Inspection Agency, or by an authorized representative of the Qualified Inspection Organization or a Certified Individual, as applicable under the associated certification program.
2.5 The reapplication of the mark shall be documented on a data report form which is retained as required for the original data report.
3 Reapplication of the certification mark shall only be provided to restore evidence of original compliance with the standard. Reapplication of the mark shall not be applied for certification of the current condition of the item or for certification to different requirements than originally constructed.
