repaired 7" hole in concrete shear wall

did the 7"hole in concrete shear wall and cut the rebar and also repaired that hole by added two 12mm rebar in each direction with epoxy and fill concrete back

need to make report to sign off as engineering point of view

what should put in exact words in report

You need to say what you did - how you know it is safe - and what you used to come up with demands for it to be safe under

If the work is already done - you should have all of this information already. If not, I'm thoroughly confused
 
We need to know the type of loading on the wall and the location of the hole. Depending on the wall, a 7" hole (specially if circular) would not be a concern, and, except for fire rating could be left 'open'.

It is far from clear what you mean. Are you saying you drilled a 7" diameter hole in the shear wall and accidentally cut a rebar? You then added 2-12M bars horizontally and vertically to make up for the missing steel. How long were the added bars and where are they located?

You need to prepare a sketch showing precisely what you did. At the moment, it is a mystery. A picture is worth a thousand words.

The exact words are going to have to wait until you provide a little more clarity.

 
and this does not sound like a "student" question. Should be in one of the Civil Eng or Structural Eng forums.

do you not have a senior person locally that can review the "repair"?
