Reuse material pipe and fitting

How many times should old pipes and fitting be used? What standards did it appear in?
How can we fix the defect weld?
Repair welding has happened more than 2 times now.

There is no limitations on the reuse of the pipes as long you follow the following rules: I am quoting ASME B31.3 code rules. You need to check what your construction code says.
1. Unknown material - Not allowed.
2. Reclaimed material: May be used provided they are properly identified conforming to a listed standard in the code or published specification, Sufficient cleaning, and inspection done to determine the minimum design wall thickness; and freedom from imperfections that would be unacceptable for the intended service.

Once is usually enough. More than that and you'll be grilled about why your repair didn't work and it will look like a poor excuse when you blame it on the materials time after time.

 
To use the piping parts if they have the proper records of fabrication, testing, inspection, etc.
The defect welds need to be fixed with the qualified welders and to follow the proper procedure and inspection.
And, you may need to find out what's the actual reason of the weld defects happened two times.
 
Where I have seen people reuse pipe they take a conservative approach.
All documents clear back to MTRs must be in order.
Any repaired area or questionable area is cut out, period.
If those two requirements prove too much of a constraint then dump it all.
