Risk Assessment Matrix Question

We are working on a project and just finished the DFMEA. The QE suggested that we use the Risk Assessment Matrix. I understand the basics of the matrix, and I understand you put new or existing parts/systems in the matrix and capture the risk of these items.

Well, I have not done this for a long time. So, this is where I need a refresher course on. What is the most common practice to go through the practice? What I mean is where do you pull the risk information from, DFMEA, FEA, etc? How does one determine that the part/system is at risk? (I use the DFMEA for the major parts/systems which are at risk and wonder if there is suggestions on other sources) And, I would like maybe a reference or a site that might have an example of how a company does their Risk Assessment Matrix.

Any of these methodologies are really just ways of capturing knowledge (or revealing the lack thereof). SO it is your judgement call as to how you want to assign risks. You are asking in such a non-concrete way that I find it difficult to answer more specifically.
