Sap2000 modeling

I am modeling 3D steel frame and used 3D analysis, standard solver after running the analysis I got error in many joints like lost accuracy 13.2 digital and some with error diagonal<0
Note I released all beam ends and have bracing and all the supports are restraint u1 u2 u3
What is wrong with my model?
