Scheme for Bridge and Road Approaches

We have this problem for the scheme of the bridge, to get the optimal length and height.

I have some clarifications on the allowable/standards/usual guidelines in the design.

1. What is the maximum grade/slope for road approaching the bridge? (excluding the approach slab and additional minimum vertical curve after/before the approach slab) Is it 7%? or 12%? This is for mountainous areas.

2. What is the optimal span/length of bridge for this situation? (Just an approximation)

There's no attachment.

The maximum grade is a function of the roadway type, geometry, vehicle type etc; for example, in NY, the maximum grade for interstate highways is limited to 5%.
