Do you use just the stems of double tees when calculating the shear capacity of the section? - can you include the concrete topping in the overall depth and area when calculating the shear capacity?

According to example 4.3.3 in the PCA design handbook (fourth edition) they use the topping in calculating d.

Do you still only take into account the stems for the shear capacity calculation? - do you disregard the "flange" sections of the double tees when calculating shear capacity?

Yes, for shear calcs in concrete, you never use the flanges.
