SPDES permit report

I am working on an industrial project located in upstate New York and have been asked to write a report to go along with the SPDES permit application form. I intend on putting detailed information about the path of the water from when it enters the faciltiy to when it will be discharged and some information about how the wastewater will be treated. Other than that, I am not sure what else would be neccessary. Does anyone know if the NYSDEC has specific requirements for this or where they can be found.

When it's time to contact DEC, I suggest speaking with someone in the "Permits" section first since the application will be subject to SEQR.
