I am in the process of creating a procedure and a checklist that should handle startups after abnormal shutdowns in our chemical plant. I need to have a multidisciplinary team sign off authorizing the startup.
We will consider as "abnormal shutdowns" only those that are historically in-frequent, like significant mechanical, electrical or quality issues, power outages, significant equipment damage, etc. It will not handle shutdowns due to SIS trips, relief valves or rupture disks blown, etc (these are emergency shutdowns based on our definition).

Can the group give me ideas on what questions I should have in my Startup Checklist After Abnormal Shutdown?
