I am in the process of creating a procedure and a checklist that should handle startups after abnormal shutdowns in our chemical plant. I need to have a multidisciplinary team sign off authorizing the startup.
We will consider as "abnormal shutdowns" only those that are historically in-frequent, like significant mechanical, electrical or quality issues, power outages, significant equipment damage, etc. It will not handle shutdowns due to SIS trips, relief valves or rupture disks blown, etc (these are emergency shutdowns based on our definition).

I thought about questions related to
-line brakes
-WorkOrders completed
-interlocks by-passed
-MOCs implemented

Can the group give me ideas on what questions I should have in my Startup Checklist After Abnormal Shutdown?

Do you have a checklist for startups after planned maintenance? I would copy most of those..

some things (may or may not be applicable) come to mind: open ends audit, critical insulation reinstalled, condensate drained to reduce hammer risk, LOTO cleared..
 
You must have a "Recovery from Abnormal Shuutdown" section.

Your plant "knows" what a normal shutdown condition is, and how you got there. (Vented tanks, cooled down, insulation not remvoed, flanges not broken, liquids all drained, steam system cooled down, etc, right? )
You know what a "normal startup" from a maintenance outage is: Fill everything, check electric lineups, checks drains, steam traps, heat up the boiler, vent traps, discharge waste product (product not meeting spec's), etc.

So, what happened during the emergency? What broke, what spilled, what DIDN'T spill or drain that cooled in place, what got plugged up or froze in place, where is the undrained condensate sitting waiting to become a water hammer, where is cold product not reacted or partially reacted? During the emergency repairs, what got broken, what else was loosened, what was tagged out or isolated that NOW needs to be checked with a one-man or two man lineup? What electronical programs/warnings/alarms need to be reset? And what electronics need to be vented or protected (vacuum gages or controllers not isolated properly?)

Before you startup to normal, you MUST figure out what the CURRENT ABNORMAL conditions are.
