Steam leak from casing split line causes

I know a few causes that might cause a steam leak from the casing split line, but I need all of them to better trouble shoot the problem. Can you help?

Causes:
1) Not achieving the right torque sequence when bolting.
2) The existance of steam cuts on the surface of the split line.
3) Un-even surface (hi spots) on the surface of the split line.
4) Using wrong torque value.

I work for a company which specialises in On-Stream Leak sealing. We seal many gasket leaks without having to shut down and depressure the system to avoid unwanted shutdowns. If you would like more informations let me know.
