The precast engineer included 3 stir ups around their 2’-7” anchor bolts which are placed in pile caps with grade beams running continually throughout. The cip installer did not install the stir ups because of limited accessibility. Pile caps vary in size but roughly 4-5’ deep. Any recommendations? Or are the stir ups really needed?

If they're detailed on the plans, you should assume they're required unless you have documentation from the design engineer that they're not. Despite what some may think, engineers aren't in the habit of just throwing reinforcing in for the heck of it.

With that in mind, if the design engineer's impression of your competence is important to you, I'd advise you not even ask the question, or be very careful about how you ask...

Btw, they're "stirrups" not "stir ups"

Not to ask, but to inform the designer. Better use a deviation report, or RFI. The designer shall make the final call.

retired13 makes a good point; how you handle it depends on your role in the construction. For the sake of the "cip installer", I hope the concrete hasn't been placed, because it will likely have to be redone correctly.

If I seem unsympathetic, it's because I am. The "cip installer" should have looked at the plans before taking the job, and if he's not able to do it as shown, he should have turned it down. If you take the job, do the job right, I say.
