Two phase flow issues

Hi sir,
There is HP steam header in our plant. I have noticed water coming out from valve bonnet due to water and steam present at same time in pipe. But doesn't seems that steam should come out from valve bonnet as liquid remain on bottom side?
Similar problem found from flange gasket when steam and HC feed(in liquid state) flow at same time. Is there any case study where two phase flow had converted into accident ?

You're in the wrong forum. Red flag your post and ask for it to be moved to the chemical plant operations forum.

But you will need to provide more information. E.g. how high is HP? Materials, design pressures, valve ratings, age of the plant, type of operation etc etc. Pictures or drawings help a lot too.

It sounds like the steam is leaking out of the valve stem and condensing on the valve bonnet, but difficult to tell from your description.

If your flange ( which flange) is leaking, then tighten it up or break it and remake the joint.

None of this is unusual.

could the water be condensed steam (ie steam on the pipe side of the valve is condensing when it hits the outside atmosphere) ?
