Under Voltage / Over Voltage Protection for Transformer Feeders

Is it typical to provide undervoltage / over voltage protection to transformer feeder.
I'm accustomed to only disconnector for feeding transformer primary (it is always ON).
For breaker controlled transformer, is undervoltage (27) / over voltage (59) required? If yes, at voltage levels?
