Updating and external reference synchronization

I have an assembly with 3 sub assemblies. At the main assembly I have a parameter to control the amount of a feature I require (a hole). In each catpart of each sub assembly I have a reaction to activate/deactivate the amount of holes. The holes are in there own part body then removed from the main part body. When I change the parameter at the main assembly once, all files update. Then when I change it again, the middle sub assembly does not update. I have to activate the catpart, update, then update the main assembly again. If I do not do the remove and run the main parameter, everything update perfect.

All parameters and features are published.

Any thoughts?

What is the active part/assembly when you modify the parameter the second time? Are you sure the Top level assembly is active?

Yes. If I click the top assembly, it doesn't ask for an update. If I double click a sub assembly, the double clice the top assembly again, it will update. If I repeat this process, it asks for another update. Then it is fully updated.
 
By changing my reaction over to rule on that specifc sub assembly, it updates perfectly. It's strange that all the other ones work expcept for this one.
