Have you ever successfully used a license on this machine before, or is this a first time setup?

Do you have LUM installed on the machine for which you will be using the concurrent offline? (evidently Yes, if you've used concurrent online)

If so, is it version 4.6.7?

had LUM 4.6.8
then uninstalled it. have the i4ls.ini installed.
I can see the licenses but when I select one and click "extract" it gives me the error.

You uninstalled LUM, but have the i4ls.ini installed?

Not sure what you mean there, but you will need to have the LUM installed on BOTH machines, (or on the server and your machine) and a network connection, until you have successfully extracted the license. (which I guess you know)

The adaptor you are using to access the network containing the lum server and the adaptor the target id is bound to have to be the same otherwise you will have trouble.

Fixed.
Had to uninstall LUM and reinstall. The LUM was running as a server on the laptop. doesn't work for extraction. reinstalled as client only.. works now.
