verify slab flatness "F" numbers

I would like to specify quality control for a concrete slab with FF25/FL20. Does anyone have a reference to the proper test procedures to verify specified slab flatness has been obtained?

Undulation on the concrete surface is measured simply with a straightedge.
 
Thanks everyone. JAE, your link has some good specification language, and that is really what I was needing.
 
Make sure you check line 40 in the reference by WillisV. This section has a lot good references to standards and specifications.

 
Please also see the newest revision of ACI-117, 117-06 for an in depth review of floor flatness and some correlations to straight edge specs. Also remember that the measurements must always be made prior to release of formwork for elevated slabs and in a timely manner. Further make sure the flatness criteria you specify is correct for your application. Flatness in a general use traffic area will not be the same a traffic in a material moving lane and the methods to obtain suitable flatness will vary as well.
