The symbol looks OK to me. You also need to install permanent identification for multiple supply sources at each supply disconnect (with references to other sources). The identification requires the same permanency as the disconnecting means itself. Identify in a manner that is clear to a first responder unfamiliar with the electrical distribution system during an emergency.

I think the exclamation symbol in the triangle is appropriate. The other graphical items in your sample are only supplementary, and are not required, but may be nice if your products are going to various countries.

Have you read faq731-376 to make the best use of these Forums?
 
I agree the graphic symbol does not adequately symbolize the hazard, but the wording is also insufficient. ANSI says to (1) Name the hazard, (2) state the consequences, (3) state how to avoid the consequences. Funny how the off the shelf signs almost never meet the standard.
