Is this for new construction or in-service? There is no magic number for re-repairs in ASME (new construction) or the NBIC (in service).

well, if it was one try only, many shops would be out of business.

sometimes a welder misses the defect or puts in a new one when repairing a weld.

there is no paragraph in code saying you can repair as much as you want.

the radiographs of the repaired weld plus tracers must be submitted to the AI for review

Just a word of caution, some materials, specifically duplex stainless steel, 6Mo alloys and carbon steel that require PWHT can be detrimentally effected by numerous repairs of the same area. I strongly recommend qualifying the repair procedures using multiple grind and re-weld for these materials if the repairs are made to the same area or close enough that an area will be subject to multiple thermal exposures.
