That's vague...
Based on your 8 words sentence, barring mechanical issues and corrosion, I would say:
overfilling
overpressured
underpressured
wrong material
contamination
excess heat (from heating, reaction or heating medium leak) and its reverse
 
and you might want to add Seismic Events -- earthquakes, depending on the seismic zone your tank is in.
 
and low temperature (<-20°), high wind and snow loads while you are considering external upsets...
