Why 'check tool and holder' under operations not reliable?

Floor and wall operation, check tool and holder ON, there are still collisions with Holder.
I am on NX8.5.0.23
Fortunately the Gouge check now reports collisions..

Thanks, our software-supplier look after the problems..it caused by undetectable geometry failure..
