WRONG ROOF NOZZLE OPENING

Good evening sir,
if the construction company fails in the opening of a nozzle in a roof so they must close the hole, what's the procedure to follow?
Re-Welding, 100 % RX.....ETC ETC
What does API recommend in a similar situation?

If this is an atmospheric API 650 tank with a nozzle ( now a hole ) in a 3/16" thick supported cone roof - just put a lap patch over it and vacuum box the completed welds. Otherwise, you need to supply a great deal of additional information to get a decent response.
