Column supported from cantilever drilled pier cap

At the end of that cantilever of the cap, I'd have a hook for that top bar. (Whether it needed it or not.) In fact, I'd probably have it for all of them.

I'd also be careful as to what the rotation/displacement of the pier will do to the forces in that column. (At least account for it. You probably already know that but just thought I'd mention it anyway.)

I'd be seeking to detail the primary reinforcement like this.

This version is more exotic but probably also more constructible. It will often require that you drop your cap top steel lower than you might otherwise place it in order get the detailing squared away.
