Required cover for formed cast-in-place concrete per AASHTO is 2" to main reinforcing and 1.5" to ties and stirrups. I think AASHTO and ACI are very similar, if not identical, for concrete cover requirements.
If you're designing or evaluating a structure per the AASHTO spec, you're going to need a copy of it.
