Formwork Design Loads

Got a formwork job for a slab. Should the design loads used be the loads given on the structural set? Or should they be construction design loads.

With respect to live, the given live load is 100 psf. Construction live load could be taken as 50 psf. Which to use?

For dead load, should I use their given dead loads or should I just take the self-weight of the materials and my forms?

you use 50 psf live load to design the form. When you design the concrete (concrete cured to design strength), it needs to be able to support the 100 psf live load.

Dead load, you use dead load of the form plus wet concrete for the form design. The concrete design use their DL and concrete DL (Assuming they have not counted the concrete).
 
If you are designing to US standards, ACI SP-4 Formwork for Concrete is a great resource.

FWIW - there are many companies out there who sell/rent/design specialized systems to simplify the whole process.
