Has anyone done revamp upgrade projects changing pipe size from 16" to 24" within same existing plant layout space?
Scope is remove 8 x 16" Outlet Lines into 36" Header, and replace with 24" Outlet Lines into 72" Header.

You have inherited an extremely poor equipment layout. Three feet between pumps. That's unheard of.
Anyways, you say 6" pipe to 2.5" pump nozzle. Review line size reduction based on flow at tee split.

Your piping system termination at the flex hose would have to be properly supported and restrained to meet the movement allowance at the flex joint. You can't model a flex joint properly of any use.

Your design sounds like mechanical reinforcement for a stub-on branch connection.
A donut ring pad should suffice your needs, start with 4" wide and see.
The B31.3 guideline for the pad and weld profile is easy to calc out by hand if your not on a network. You can play with...

The answer to your question is yes, you can modify the existing piping this way to retrofit adding a small bore fitting to the piping wall exterior.
This is normal refinery service, A106 Pipe.
