AASHTO Bridge Deck Design

I'm trying to find a worked example for AASHTO Bridge Deck design.
Every example I have references AASHTO A4-1 table for the live load effects. This would be fine for the majority of deck design situations but I need to check some special permit trucks to ensure that I'm not underdesigning the deck.
I want to make sure I'm applying the truck loads properly. Thank you in advance.

Look at Section 3.6.1.3.3 of AASHTO LRFD including commentary for what loads to consider for deck design. I have not designed a deck for permit loads and I have not come across a bridge owner agency which requires that.

Thanks EQguys. HL-93/Tandem is still expected to control but this particular official simply wants to go the extra mile due to the increased permit truck volume from a neighboring state. They want to check the load effects of this permit truck that's not included in their own Bridge Manual. I would be more comfortable if I had a worked example to check the procedure against my calculations.

The attached FHWA publication is from 2003 but it has an extensive deck design example.

here's a Caltrans example from 2011. It states that permit load usually doesn't control.

Thanks Bridgebuster, I used both those detailed examples to prepare my calculations. The FHWA example used AASHTO A4-1 and Caltrans used its own proprietary software to generate the live loads. I was interested in seeing an example where a permit truck was applied to generate the live loads and whether there was any specific guidance there, but it appears most jurisdictions just ignore it altogether based on industry consensus.
