I know that it often recommended that a rubber coated or non-metallic vibrating head be used when consolidating concrete with rebar that is epoxy coated. Could not find in 309 or 318 an exact requirement for this. IS there an exact requirement for this?

I think it might then be just a recommendation from CRSI or something. Not a code issue. More a workmanship issue.

Does not matter at all. I have seen both kinds of vibration material and neither effects the coating on the rebar if used in a vertical insertion proceedure.

Take a look at the ASTM A 775 for epoxy coated bars. I believe there is a section on "Guidelines For Job-Site Practices" which recommends that equipement handling epoxy coated bars to have protected contact areas.
