In section 4.16.4.3, what does the term "Fabricated Flange" mean?

In my opinion, it means nothing. Ignore the word "fabricated". However, please submit an inquiry to the Section VIII Code Committee to get that wording fixed.

Good question - now please complete the follow-through to fix it for everyone else. That's how this Code thing works... Let us know the tracking number you are provided by ASME. There are several members of the committee(s) that either lurk or are active here. We'll make sure it proceeds appropriately.
 
IMHO it indicates the flanges that are machined from a piece of material that includes, in its thickness, the full flange (ring+hub). This by contrast with the standard flanges that are normally obtained by forging the material to a nearly finished geometry.
The concern is obviously that, particularly when using plate material to machine cut the flange, the hub might be affected, in the integrity of its attachment to the ring, by laminations in the plate.

What you said makes sense. Combined with Part 3 requirements, essentially all flanges with hub need to meet the requirements of 4.16.4.3.

I submitted interpretation request 16-2209 asking that if by "fabricated flanges with hub" it means "all flanges with hub".

Are you trying to suggest that the Codes are issued without anyone "proof reading" them? Never!!!
 
Fabricated or forged. Fabricated flanges other than forged, but don't include the ones machined from forgings.
