Blended-out Surface Damage SCF Reference

I am searching for a copy of the original reference for the information given in Fig. 16.6.1 of Prof. MCY Niu’s book – Stress Analysis and Sizing, 2nd Ed. In particular, I am looking for a copy of the reference for the blended-out surface notch (scratch, gouge, corrosion pit, etc.) per Fig. 16.6.1 (c).His response to my e-mail request was that Refs. 16.5 and 16.6 cover this information. I have obtained the AGARD AG 278 document from the following FTP site: -

This document does not have the relevant SCF data I am looking for.
I have not been able to find an SCF reference for a blended-out surface damage. If anyone has access to the relevant SCF information from the “Salvage Repair – Standard Factory Operation”, Lockheed Corp document, or an alternate source, I would be most grateful for a copy or a lead to where I can obtain a copy of this information.
