LUCKNOW, Ont. — At least six people were injured when a bridge that was under construction collapsed Saturday near Lucknow, in western Ontario.

The Ontario Provincial Police said a construction crew was working on the Eighteen Mile River Bridge on Bruce Road 86 when it came down at about 12:30 p.m., near this community about 200 kilometres west of Toronto.

Volunteer fire Captain Stephen MacPherson, who arrived at the scene shortly after the collapse, said the workers were pouring concrete on the top on the bridge when the whole structure went down.

I hear the Dept. of Labour is investigating. I wonder if they had adequate bracing up, or had high wind loads.

Sounds like maybe poor formwork/scaffolding?

 
The contractor had a penalty clause in the Contract and was about a month or so behind :(
From what I can see from the pictures, the formwork is what failed, possibly due to inadequate lateral bracing.
This happened about an hour or so from us.
