Where can I find information about how to repair a built-up plywood beam, such as glue type, nailing patterns etc?

I wouldnt rely on glue. Proprietary glulams e.t.c. are verified by testing, not calculations.
 
The APA (American Plywood Association) has a lot of information on built-up plywood members, with design examples. I would start by googling there.

It really depends on the "repair" that is required. It may need to be reinforced with steel plates or angles. Or sistered with engineered wood members.
