Depends on what it's being used for:
API 650 for petroleum
AWWA D100 for water tanks
ASME for others?
I think what your photo note is pointing to is typically called a doubler plate.

Whats the problem in detail?
 
Owner wants to add new pipe to existing tank. We do not typically do this type of work.
 
The plate can be round or diamond shaped or mousehole shaped depending on preference and weld clearance.
API-650 gives tables of standard sizes and details for shell penetrations.
AWWA D100 gives rules for reinforcing, but not as much detail. Approach is pretty much the same, though.
ASME is similar in concept but much more complicated due to the mass-o'-detail issues, so is best avoided.
Basically, though, the cross-sectional area of that reinforcing pad needs to equal the cross-sectional area of the metal removed in the hole.
For a run-of-the-mill tank, you can make that reinforcing plate same thickness as the shell, twice the nominal diameter of the opening, and that'll fulfill the AWWA rules. Full fillet around the outside, grove weld on the inside of the nozzle, fillet on the outside of the nozzle.
