what are the most suitable mean of anchoring vertical columns, on the foundation to avoid any misalignment of bolts? as we are not sure about the anchor data and BOLT CENTER DIA. The equipment is sort of column, pic attached for ref. The one in my mind is providing pocket, is there any other option as

I generally insist that anchor rods/bolts be located by means of a template. They drill holes in plywood/osb and this makes sure they are in the right location, generally. I also insist that anchor rods are weldable material so it's easier to fix is something goes wrong.

Rather than think climate change and the corona virus as science, think of it as the wrath of God. Feel any better?

If you are not sure about the anchor data and BOLT CENTER DIA, ask from the column manufacturer. They must provide the anchor bolt size, length, bolt center dia. and orientation of the anchors.



Providing pocket is not a good idea ..
 
If it is an existing column without known dimensions, column itself can be used as a template to drill holes in the plywood. Then use drilled plywood as a template to secure anchors and leave it in place until concrete is set. Most steel erectors we work with insist on pouring foundation themselves, so the anchors are right where they expect them to be. Takes all the headache and finger pointing away.
 
Is these sleeves will be refilled with concrete after fixing equipment.
Equipment will be available at the site, nearly after construction of the foundation, only hand measurement is available right now, which may leads to inaccuracy.
 
Yes, you can grout the sleeves after placement of the anchors. But it may not be physically possible depending on the configuration of the equipment base plate. It just all depends on the design goals for the anchors. Check out this discussion: thread507-474830
 
I don't know if they still make them, but there were plastic corrugated tubes that were made for forming the sleeved anchorage.
