Component Identification Help 1

I need to identify this component for a system I am working on. It is basically a switching terminal block. The two small gage wires arethe control signal, and there is a jumper on two other pins. The manufacturers symbol on the device is a D W with a diode in a circle between the D and the W. If anyone can help me identify the component or the manufacturer I would appreciate it. I need a data sheet for this device.
