Creating a simple shape

I cant seem to create a simple shape here. I am trying to create a tapered cone on a radial curve. Which is the best/simplest way to do this you think? I am using Sweep/Swept to do it but gives an undesired flat section in my cone shape. See attached JPEG for sketches created to attempt this. Any suggestions?

To an Engineer, the glass is twice as big as it needs to be.
 
Yep, John, your the man! I got it to do the same thing, I had to divide my two diameter sections into two sections. Thank you.
