design cast in place piles under lateral load

Attached are a couple of pages from I piece I found in my files. Other pages are apparently misplaced. I wonder if anybody recognizes the source of the document and could tell me where to obtain a copy. Also I wonder if anybody could suggest another source of possibly r a more friendly piece for designing cast in place piles under lateral loads.

Thanks for that sliderule. I appreciate your reply. I can print the text nut the diagrams won't come ... not even with screen shot.

BTW I took all my training in engineering college using a slid rule. No such thing as computers then LOL
