I am going to be giving a presentation to first year graduate engineers about the construction administration phase of a project. In order to provide concrete examples on the importance of the CA phase I was hoping to use some specific example of when a major design flaw was caught or created during the CA phase.
The one example that I can think of at the moment is the Kansas City Hyatt pedestrian bridge collapse in which a detail was modified and eventually approved during CA that led to the failure.
