Developing a custom procedure for special inspection

I have been asked to develop a special inspection procedure some something simple (a wedge anchor) that has been post-installed in concrete.

The "what" is besides the point, but I cm curios if there are any formats or templates for something like this, or should I just write out the procedure in a letter?

I think the "what" is fairly important. For example, with the wedge anchor, first thing I would do is review the manufacturer's literature and see what they have to say about installation instructions, common problems, and how to identify correct/incorrect installations. In addition to that, the application is important - inspection of a wedge anchor securing a new set of stairs vs securing a fall arrest anchor is going to change your inspection procedure....there may be some governing codes/regulations that will dictate a portion of your inspection procedure.

Aside from the "what", I'd try to keep in the inspection procedure clear and simple, preferably in list format. Leave space for notes (might want to record why the item failed or passed) and provide a standard way of identifying the inspected item - to use your wedge anchor example - provide a space to list the anchor # (or some other identifying tag), its physical location, date inspected, name of inspector, purpose of inspection etc...
 
Check the ICC-ES report for the specific anchor to be inspected. I believe it will list the items that require inspections. I believe with most post installed anchors, the first anchors installed on a job require continuous inspection of the entire installation procedure, including drilling and cleaning the holes. After the initial installations, I think subsequent installations of the same anchors on the same job can be periodic. With periodic inspection, all you can really inspect is size and location, and manufacturer if they have some kind of ID mark on them.
