Besides CSB, I'm looking for more examples and case studies of a failure/accident in the oil, gas and chemical industry that was due to a poor integrity program and/or inspections.

USA Pipeline related failures are catelogued at the PHMSA Web site.
Canada also has a similar website Canadian Transportation Safety Board, or something.
