Hi. "Cosmetic Flange" related to a PSV or Safety Relief Valve.
What it means is that the flange has the dimensions and profile and/or the ability to fit the designated flange rating but is NOT FULLY RATED to the designated pressure class.
Example: A 2" x H x 3" safety relief valve can have a 300# inlet flange fully rated and 150# standard outlet flange.
For whatever reason, the end user requires the outlet to be 300# maybe to mate up with existing pipework.
The manufacturer would then supply a non standard 2 x H x 3 300# x 300# COSMETIC OUTLET
This means that the manufacturer has taken his standard 300 x 150 body casting and machined the outlet to 300# dimensions - in many cases the outside diameter will be less than 300# but it does not matter since the pressure is restricted to 150# class (limited by API-526 or valve design). Some manufacturers have enough material on a raw body casting to actually maintain a 300# machined outlet. Most however will not and where requested, the manufacturer will get his body casting modified to allow full sized 300#. This option is not fully rated since the body (wall thickness) governs the outlet rating. In all of these examples, it can be said that the outlet flange is cosmetic only.

Per ISO, only the term Safety Valve is used regardless of application or design.
