Is anyone aware of FH Lag Screws? I've only encountered hex headed ones.

Nope. Whenever I've needed something like that, I've detailed a counterbore, large enough to accommodate a plate washer and a socket.
 
I'll suggest #14 screws, designed as screws, not lag screws.

Most timber screws have flat heads. Will they work?
Simpson SDWS for example.
 
Thanks, they would work... the fastener is used for handrail mounting plates and the Arch has spec'd 1/4"x1" FH lag screws... I've designed them using screw design for #14 x 3" wood screws (I'm comfortable with that). The 1" lag doesn't even come close... I'm sealing drawings for steel fabricators... and add my own notes to get things to work. I'm not required to, but I generally look at spec'd details to work.

Wood screw mounting for handrail seems wrong. Is the wood also pressure treated lumber? Maybe screwed into end grain?
 
I just stipulate conditions and criteria for fastening... and have the EOR/Arch to confirm... Most fastening to wood, I've encountered with handrails is underdesigned... I stipulate requirements for it to work End grain, side grain, spacing, lumber species, grade, etc., min thickness and require the backing is secured to the structure.
