Does anyone have a sample Florida Statement of special inspections form, or know where I can find one on the web?

Or is my typical form from my region of the country (north east) acceptable in Florida?

The linked form is for Special Inspections under the Building Code provisions and as originally defined under the Uniform Building Code with Florida's adaptation. This is different than a "Special Inspector of Threshold Buildings" under Florida law. Though they are similar, they have different meanings, implications and requirements.

I'm looking for the form to fill out that tells the inspector what to inspect (and file a copy with the building official). In my area, each state has a little different look to their typical form. I don't want to use my template and submit it to the building offical only for him to look at it and not recognize it because the Florida form is different.
 
LPPE...In Florida, the "form" you are describing is called a Threshold Inspection Plan, developed and filed by the Structural Engineer of Record. To my knowledge, there is no form for this and each SEoR has different ways of doing it. Ask the Building Department in the locale of the project to provide you with an example of one having a similar scope to your project.
