I am not sure if this kind of thread is already existing so I just wanted to start one. Actually the company I work for has a program rolled out for reporting any design lapses related to safety, health and environment. The program is called "Zero harm" or "serious about zero. Employees have to report their observations related to safety in engineering and design. It could be some small safety related incidents in the operating plants which you come across or near misses. The purpose for this is to enhance awareness about safety.
I hope I was able to make my point clear. I will welcome any suggestion/advice.

advice on what? You didn't state a question.
Safety is always good though.

Good way to get everyone involved in process improvement.

I just wanted to ask question like:

Has any one come across a situation where in an engineering construction package, HSE risk reduction methods not adequately considered in design. or
HSE aspects for construction, operation, or maintenance not adequately considered

Sure, I mean we've had to tweak numerous designs because we didn't have a complete understanding of the relevant regulatory compliance requirements - or sufficient attention wasn't paid - during design.

The process you describe is called "risk management". It is usually uniquely tailored to whatever industry is being served, and is often enhanced (bogged down) by published standards.

In the medical electrical equipment field, its chief purpose is to increase the stress level and blood pressure of the project manager. Sorry, been a looooooong Monday.

It is better to have enough ideas for some of them to be wrong, than to be always right by having no ideas at all.
