I am not sure if this kind of thread is already existing so I just wanted to start one. Actually the company I work for has a program rolled out for reporting any design lapses related to safety, health and environment. The program is called "Zero harm" or "serious about zero. Employees have to report their observations related to safety in engineering and design. It could be some small safety related incidents in the operating plants which you come across or near misses. The purpose for this is to enhance awareness about safety.
