Help with basic wiring of low volage 1/3 hp motor

Hi, I purchased a motor yesterday and would like some help wiring it for 115 v.

Am I correct to assume that if I use an extension cord with three wires (one being ground) one hot wire goes to p1 and one hot wire goes to t2,t5, t4? And where would I attach the ground wire from my cord?
