Hole cut size/Nozzle Fit-up

The shop I work for is currently had this question came up about; is there anything specific in Code that says how big a hole can be when compared to the nozzle that will go in that hole. For instance a 2" pipe, OD is 2.875" the hole diameter that was cut is say 3.5" is there anything in code that says you just cant weld that up with that big of a gap. I know in our case some welders would try and some would notify someone of this situation...

Well, first, 2" pipe is 2.375" o.d. but it really doesn't matter.

i don't remember this being addressed in code. BUT, the gaps at welds must meet your wps.
Most wps do not stretch out to 7/16" weld gap.
 
However, you can build up the hole with weld metal, NDE (hopefully) and then re-bevel etc as required. Approved procedures of course.
