Question 1:

Are the horizontal ties (i.e. ladder ties) located between the faces of CMU block spaced between every block or at a standard spacing?

Question 2:
What is the standard gauge size for horizontal ladder ties?

Thanks for your help.

I usually space them at 16 inches. There's nothing from spacing them more or less often as long as you meet minimum steel ratio required by code.
Standard size we use is 9 gage wire. There are other gages made.

Almost all cases in the US they are spaced at 16" o.c.

In our area, spacing of the horizontal "Durrowall" reinforcing is commonly at 16" on center, every other block course. However, it has also been a common practice, in this area of the Midwest US, to install the Durrowall in every course for below grade masonry walls.
